We are seeking a dynamic and experienced Primary Care Network (PCN) Pharmacist to join our multidisciplinary team. The successful candidate will play a pivotal role in optimizing medication management and delivering pharmaceutical care to a diverse patient population across our network of general practices. This position offers a unique opportunity to influence and improve patient outcomes at the community level.
Key Responsibilities:
– Conduct medication reviews and clinical audits to ensure effective and safe prescribing practices within the PCN.
– Provide expert advice on medications to GPs, practice nurses, and other healthcare professionals within the network.
– Develop and implement protocols and guidelines to improve prescribing efficiency and medication safety.
– Participate in multidisciplinary team meetings to discuss and manage complex patient cases.
– Lead the management of chronic diseases through medication optimization and patient education.
– Support the integration of pharmacy services across the PCN, facilitating a collaborative approach to patient care.
– Deliver training and mentorship to pharmacy technicians, GP practice staff, and other healthcare professionals.
– Engage in public health initiatives, health promotion activities, and patient education programs within the community.
– Manage projects and initiatives aimed at improving the quality of pharmaceutical care across the network.
Qualifications and Skills:
– Master’s degree in Pharmacy or equivalent, with current registration as a Pharmacist in the applicable jurisdiction.
– Postgraduate qualification in Clinical Pharmacy, Prescribing, or a related field is desirable.
– Proven experience in a primary care or community pharmacy setting, with a strong understanding of primary care clinical services.
– Excellent clinical skills and knowledge of medication management in chronic diseases.
– Strong leadership and project management abilities, with experience in driving quality improvement initiatives.
– Effective communication and interpersonal skills, with the ability to work collaboratively across different healthcare disciplines.
– Proficient in using electronic medical records and clinical decision support tools.
